When do you notice the problem (time of day, time of year, weather conditions)?
Where do you see the symptoms on the plant (top, bottom, inside, outside, root area)?
What part of the plant has the symptoms (leaves, stems, trunk)?
What symptoms do you perceive (sight, smell, feel, hear)?
What color, shape, size are they?

What is the history of the area around the plant?

Has there been (digging, grade change, construction) nearby?
Have any plants, buildings, pathways been modified nearby?
How have you (or someone else) been caring for the plant or those nearby (watering, fertilizing, pesticide applications, weeding, and pruning). Please include any details such as how often, how much and products used!
Has the plant been transplanted, planted by seed, sod or just planted in its present site and how long ago did this occur?

What is the environment that the plant lives in?

How well does the soil drain and what type of soil is it (clay, rich, sandy)?
How far apart is/are the plant(s) spaced?
How much sun does the plant receive each day (number of hours, intensity, morning/afternoon)?
What is the area like (exposed, near a road, near a building, near a pool,
downspout or a neighbor’s yard)?
Do people, pets, vehicles walk or drive in the area around the plant?
Is there landscape fabric, plastic and/or mulch around the plants?
How deep is the mulch, what type of mulch is it and
how far does it extend around the plant?
